Here is a storyboard template:

Blocking Diagram — there is not really a template for a blocking diagram.  It includes the details of a space, where your subjects will be, and where you can place cameras to get the shots you want.  You can also add lighting.  These are more common in fiction production, but are useful any time you are working with several people in a space. Here are a couple of samples:
